A patient's own T cells, an essential part of the immune system, are genetically modified through a complex process known as CAR T-cell therapy. Leukapheresis, which involves removing T cells from the patient's blood, is the first step in the treatment. In the lab, these T cells are subsequently genetically modified to express a chimeric antigen receptor (CAR) on their surface. The CAR is a synthetic fusion protein that combines intracellular signaling domains that activate T cells upon antigen binding with an antibody-like domain that targets a particular tumor-associated antigen (for example, CD19 for B-cell malignancies).

 

CAR T-cell treatment has shown impressive clinical results, especially in hematologic malignancies. The first CAR T-cell therapy, Kymriah (CTL019), was licensed by the U.S. FDA in 2017 to treat pediatric and young adult patients with relapsed or refractory B-cell ALL. Other CAR T-cell treatments, such as Yescarta for large B-cell lymphoma, have been authorized in later years for a variety of B-cell cancers. These treatments have shown full remission rates in some patient groups, including individuals who had not responded to previous treatments.

 

A new and exciting method to immunotherapy, chimeric antigen receptor T-cell therapy gives patients with tumors that have not responded to conventional treatments hope. Even if there are still obstacles to overcome, especially when it comes to solid tumors and managing toxicity, the continuous developments in CAR T-cell technology have the potential to revolutionize the way cancer is treated. With further research, CAR T-cell therapy could soon be a key component in the fight against cancer, providing effective, individualized, and long-lasting treatment choices.
